The Fight to Protect Compounding and Patient Access Continues.

In 2026, APC, state associations, pharmacists, patients, and other advocates stopped or mitigated major legislative threats to pharmacy compounding in 10 states. For 2027, APC anticipates similar proposals in as many as 25 states, plus at least two bills moving through Congress.

In 2026, major drugmakers backed and promoted a coordinated wave of restrictive compounding legislation in statehouses across the country, most notably targeting compounded GLP-1s. These bills sought to impose new barriers on lawful pharmacy compounding that threatened pharmacists’ ability to meet individual patient needs and pushed restrictions that would do nothing to stop the counterfeiters and bad actors that supporters claimed were the target.

2027 Is Going to Bring the Toughest Fight Yet

APC anticipates similar proposals, possibly in as many as 25 states, that threaten pharmacy compounding and patient care.

What you can do

Real Solutions for Patient Safety

These drugmaker proposals do not target real concerns that have arisen from the era of GLP-1s. APC proposes five practical reforms aimed at real concerns. Many of these solutions could be pursued at either the federal or state level. These are common-sense solutions that actually enhance patient safety.

  1. Clear Labeling of Compounded Medications

    Require clear disclosure on medication labeling that compounded medications are not FDA-approved.

  2. Adverse Event Reporting

    Require 503A pharmacies to report serious adverse events via FDA’s MedWatch system.

  3. Restrictions on Unlawful Consumer Marketing

    Prohibit direct-to-consumer marketing of substances that cannot lawfully be compounded for human use. For instance, research-grade ingredients are illegal for use in human drugs. Marketing them to consumers should be, too.

  4. Stronger Oversight of Medical Spas

    Close oversight gaps involving medical spas and similar businesses at the state level, where they should be regulated.

  5. Protection from Deceptive Legal Tactics

    Protect lawful pharmacies from deceptive legal intimidation by drugmakers.
